ALCOHOL DRUG EVALUATION or SUBSTANCE ABUSE EVALUATION | |

This type of evaluation is needed in cases where the Court, Probation Officer or other entities require you to undergo an evaluation other than the DUI evaluation. Sometimes this may be done after you have completed the required class hours to see if additional treatment is needed. This evaluation is more in-depth than the DUI evaluation, but uses the same DSM IV Diagnostic tool. All of our clinical type evaluations, consist of a set of comprehensive questions designed to determine whether the offender meets the diagnosis of substance abuse or addiction, or has misused the substance that led to the arrest. To determine such diagnosis the Assessment Counselor uses the DSM IV Diagnostic tool which is also used by the American Psychiatric Society. Depending on the history of the use, previous treatment experience and diagnosis the Assessment Counselor will make a recommendation that will satisfy the requirements of the State where the offense occurred.

| Supporting documents must be received within one week . If there are special circumstances that time frame can be negotiated at the discretion of the counselor. If you are unable to obtain a document listed below, please contact us to see if your evaluation can still be completed. If not DUI related but involves an arrest * If the Alcohol Drug or Substance Abuse evaluation is being completed for the purpose of getting your license reinstated due to a DUI then the following documents are needed. Notice of Summary Suspensions, or The following forms can be downloaded from this site: http://www.rightdirectionservices.com Collaborator Interview (must be completed by someone that has known you well for at least 2 years and who can support what you are telling the counselor about your Alcohol and/or Drug use history. Release forms ( for each person/entity you authorize us to talk to about your evaluation such as the court, Probation Officer, Attorney etc) Payment Authorization Form (this form is the written authorization for the payment that you or a third party made for the evaluation. The payment was made through PayPal who processes all of the payments we receive but we do not receive any documentation with signature that says that this was actually an authorized payment. Due to the increase in credit card fraud it is neccessary that we have this document on file. It will be shredded after 6 month.) Please let us know asap if the your State has specific forms that need to be completed by the agency who prepares your evaluation. Completion of such a form is not part of the evaluation and are subject to additional charges which can range from $25 - $80, depending on the complexity. |

Alcohol Drug Phone Evaluations This is the standard Alcohol Drug Evaluation done via phone. The evaluation process includes filling out an online questionnaire. Upon making payment you will receive an auto-responder instruction letter with various links to forms, including the questionnaire. Once you submit your questionnaire and the required documents you will be contacted by the assigned counselor to schedule the phone interview. Once all documents are received and the interview has been completed you will receive the finished evaluation within 2 business days. |

Alcohol Drug Skype Evaluations The Skype Alcohol Drug Evaluation is a live meeting via the internet using Skype Video which is a free program. You will need a webcam which is inexpensive and easily installed. The evaluation process includes filling out an online questionnaire. Upon making payment you will receive an auto-responder instruction letter with various links to forms, including the questionnaire. Once you submit your questionnaire and the required documents you will be contacted by the assigned counselor to schedule the Skype Evaluation interview. Once all documents are received and the interview has been completed you will receive the finished evaluation within 2 business days.. |

Weekend Alcohol Drug Phone or Skype Evaluations (Weekends or Holidays) Weekend Alcohol Drug Evaluation requests will be completed faxed and emailed to you or your designated party the same day providing all required documentation has been received. Your request for the Weekend Alcohol Drug Evaluation, must be received no later than 2pm CST on Sunday and all documents must be received by 4pm CST. You must also be available to be contacted by the counselor for any additional information neccessary. |
