DUI EVALUATIONS | |

DUI Evaluations are generally Court Ordered and consist of a set of comprehensive questions designed to determine whether the DUI offender meets the diagnosis of substance abuse or addiction, or has misused the substance that led to the DUI arrest. To determine such diagnosis the Assessment Counselor uses the DSM IV Diagnostic tool which is also used by the American Psychiatric Society. Depending on the history of the use, previous treatment experience and diagnosis the Assessment Counselor will make a recommendation that will satisfy the requirements of the State where the DUI offense occurred. | |

| Supporting documents must be received within one week . If there are special circumstances that time frame can be negotiated at the discretion of the counselor. If you are unable to obtain a document listed below, please contact us to see if your evaluation can still be completed. Notice of Summary Suspensions, or Officers Sworn Report, or any legal document that contains data of your arrest including the BAC or Refusal Police report Drivers Abstract (Driving History for court purposes) Copy of Photo ID or Drivers License Print Out of All Previous Offenses (if any) BAC or Lab Work Results (if not refused) *any additional state forms Forms can be faxed, scanned and emailed, or mailed . Phone DUI Evaluations This is the standard DUI Evaluation done via phone. The evaluation process includes filling out an online questionaire, and then being interviewed via phone by the Counselor. The interview process takes about 1 hour and you will receive your completed evaluation within 3-4 business days provided that all required documents have been submitted. |

Skype Video DUI Evaluations The Video DUI Evaluation is very similar to a face to face evaluation and is conducted via Skype Video which is a free program. The evaluation process includes filling out an online questionaire and then the video interview by the Counselor. The interview takes about 1 hour and you will receive your completed evaluation within 3-4 business days provided that all required documents have been submitted. |

Weekend DUI Phone / Skype Evaluations (Weekends or Holidays) Evaluations will be completed and emailed to you or your designated party the same day ( by 11pm CST) providing all required documentation has been received. Your request for the Same Day DUI Evaluation must be received no later than 2pm CST on Saturdays and Sundays and all documents must be received at that time as well. The process of the evaluation includes completing a online questionnaire followed by the phone interview with the counselor. You must be available to be contacted by the counselor to contact you to set up at time for the phone interview. If you are NOT available at certain times because you are working etc be sure to email this to support along with a time you can be reached. The phone interview must be completed no later than 4 pm CST in order to be completed by 6 pm CST. If you need the evaluation at a specific time please email to support so that we can make sure we can meet that deadline. |
